The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services are displayed in the surgery.
There are several services, such as the signing of certain forms which are not covered under the National Health Service, such as:
- Private Medical Certificates
- BUPA and Private Insurance Forms
- Holiday Insurance Claims
- Examinations for Driving
- Shotgun Licence Forms
- Sickness Benefit Forms
You will therefore be asked to pay for these services. A list of charges is also displayed in the reception area. Please note that special appointments are required for all the above medicals.

Private Medical Letter
When your doctor is asked to give medical information about you in the form of a report, letter or certificate, the request kick starts a series of processes.
This takes time and is not always straightforward or simple to complete. Some of the information is not available easily and will mean the doctor has to sort and select the right information for the request.
The doctor also must establish who is funding this work and if it is not part of their NHS work, agree a fee for this.

Private Medical Examinations
Medical examinations for special purposes such as pre-employment, fitness to travel, elderly drivers, fitness to undertake sports etc, will require longer than a routine appointment.
These examinations do not form part of NHS Services, you will be charged for any such examination. Please check the fee chargeable when you require to arrange any such examination.
